Solid Foundation Counseling Services is an approved provider for the South Carolina Departments of Probation, Parole, and Pardon Services.
The Department of Probation will forward us a Service Referral Information Form (Form 1054). Once we receive your referral, we will attempt to contact you for approximately 21 days. If we are unable to reach you and schedule you for an appointment, we will return your referral to your referring agency.
You have the option of starting your enrollment process today by submitting a ‘New Client Information Form’. We will review your submission and if we do not have a Service Referral Form on file, we will contact your probation officer for a copy of your referral.
Your forms will be reviewed and if any questions remain, we will contact you. If all information is provided, we will forward your Initial Enrollment Forms to you. Forms are sent electronically to the phone number or e-mail you provided to us. The forms can be accessed through a secured web-based link and signed electronically.
Initial Enrollment Forms will be sent within 2 business days.
After we receive and review your Initial Enrollment Forms, we will contact you to schedule your first appointment. Your appointment and fees will depend on the services you need. In order for us to understand which services you need; we review the Service Referral Information Form (Form 1054) that is submitted to us by your referring agency.

We do accept some health insurance policies. If you have a health insurance policy, it may provide some coverage for behavioral health services. Not all programs are eligible to use health insurance policy for payment. Insurance companies normally only pay for services that are deemed “medically necessary.” In therapy, medical necessity is established by diagnosing a client. Furthermore, we have to demonstrate that a client’s mental health condition is affecting and interfering with their overall functioning on a daily basis in a “clinically significant” manner.
If you have a health insurance coverage with a plan we accept and would like to use your health benefits for payment, the following will apply:
