The Solicitors Office will forward us a Service Referral Form. Once we receive your referral, we will attempt to contact you for approximately 21 days. If we are unable to reach you and schedule you for an appointment, we will return your referral to your referring agency.
You have the option of starting your enrollment process today by submitting a ‘New Client Information Form’. We will review your submission and if we do not have a Service Referral Form on file, we will contact your case manager to obtain a copy of the referral.
Your forms will be reviewed and if any questions remain, we will contact you. If all information is provided, we will forward your Initial Enrollment Forms to you. Forms are sent electronically to the phone number or e-mail you provided to us. The forms can be accessed through a secured web-based link and signed electronically.
Initial Enrollment Forms will be sent within 2 business days.
After we receive and review your Initial Enrollment Forms, we will contact you to schedule your first appointment. Your appointment and fees will depend on the services you need. In order for us to understand which services you need; we review the Service Referral Form that is submitted to us by your referring agency.
Your initial appointment may be a Comprehensive Evaluation or an Orientation session.
Prior to enrolling into your treatment program, you are required to attend a in-depth Behavioral Health Screening evaluation to determine appropriate treatment services. This evaluation helps identify any concerns that you may need help with. It may be beneficial for both you and your referring agency to understand the nature and cause of any difficulties affecting your daily functioning so that appropriate recommendations and treatments may be offered. Evaluations will be administered with structured clinical interviews and psychometric instruments, such as:
· biopsychosocial assessment
· an alcohol and drug assessment
· a hostility/aggression assessment
· a risk assessment
· drug screening
Following your evaluation, you will be scheduled for individual, group, or a combination of individual and group sessions. The frequency of treatment sessions, number of sessions, goals, type of counseling and any alternative counseling methods, will depend on several factors, to include: your current circumstances, your overall goals, and any guidelines that may be placed on you by the court system, legal system, and/or Department of Social Services.
